Please complete this Authorization Form, and include all required documents,  when you are not be able to present yourself in person at THAI when issuing Award tickets, paying applicable taxes, surcharges or service fees and registering or changing Award Nominees.

 

Letter of Authorization

on my behalf shall have the same affect as acts of my own.
